SUMMARY:Webinar: Your role in advance care planning: measuring outcomes one year after the new Advance Health Directive release
DESCRIPTION:Join Dr Jacquie Garton-Smith and Dr Mariam Bahemia for an interactive discussion focusing on use of the updated Advance Health Directive (AHD) in WA. There is currently low uptake of AHDs across Australia\, including WA. Increased uptake of Advance Care Planning (ACP) and completion of AHDs is expected to reduce the disparity between treatment preferences and delivered care. This session will explore the role of GPs in advance care planning\, including supporting patients to prepare AHDs. We will also share a new mini-audit activity for ACP and AHD that can be used to review whether opportunities to introduce ACP or AHD conversations into consultations\, assessments and care plans are being maximized. \nOptional completion of the mini-audit will earn GPs 6 Measuring Outcomes CPD hours. \nThis webinar is delivered in partnership with the WA Department of Health. \nRegister here

We will also consider the impact of dementia on informal caregivers and their role when the person living with dementia is ill. \nIn this session we will: \n\nReview dementia types and characteristics as a life-limiting condition\nExplore what it means to be a person\, implications for independence and stigma\nConsider the impact of illness on the person living with dementia\nReflect on informal caregivers’ role and need for support\nLearn the principles of person -and relationship-centred care\n\nThe workshop will be presented by Davina Porock\, founding Director of the Centre for Research in Aged Care at ECU’s School of Nursing and Midwifery. Her research program focuses on living and dying well in residential aged care\, with a particular interest in person-centred practice and recognised dying. \nThe cost of this workshop is $22.
