Early diagnosis of heart failure and effective management in primary care prolongs lives, keeps people out of hospital, and improves quality of life. Managing heart failure is complex. High rates of mortality and hospital readmission make this a challenging condition for patients and their GPs. But there are clear principles to support effective management in primary care.
This educational visit supported by NPS Medicinewise and the Heart Foundation is part of a multifaceted program designed to:
• support GPs in the early diagnosis and classification of heart failure
• provide clinical guidance on initiation and ongoing management of medicines
• empower patients with effective self-management strategies to improve their quality of life.
Educational visits are available as:
• One-on-one tailored discussion for GPs (30 minutes)
• Virtual visits via video call, so GPs who find it difficult to schedule an in-practice visit can still benefit from our visiting program
• Small group meeting for up to 10 health professionals – GPs, practice nurses and and pharmacists (1 hour).
This activity has been approved for 2 points (CPD Activity) under the RACGP CPD Program and accredited for 1 hour under the educational activity category in the ACRRM PD Program.
