South West Chronic Conditions Collaborative (SWCCC)
The SWCCC oversees the development, implementation, monitoring and evaluation of strategies which work towards integrated systems and shared approaches to services and programs for consumers with chronic conditions, including the use of technology. Their focus is on Chronic Obstructive Pulmonary Disease, Asthma, Chronic Heart Failure, Diabetes and Chronic Kidney Disease.
Membership of the SWCCC includes representatives from WA Primary Health Alliance, WA Country Health Service, GP Down South, South West Aboriginal Medical Services and community pharmacist, David Williams.

South West Health Research Collaborative (SWHRC)
The SWRC is a newly developed initiative facilitated by the Lishman Foundation made up of a community of health professionals, researchers, Universities and other key stakeholders to optimise the opportunity for research in the South West.
The SWRC is a research consultation forum that aims to identify current research activity, propose future research activity, and identify and promote translational research priorities.
Current membership of the SWRC consists of the Lishman Foundation as facilitators, Rural Health West, St John of God Healthcare, WA Primary Health Alliance, WA Country Health Service, and the Rural Clinical School.
South West Aboriginal Health Planning Forum (SWAHPF)
Regional Aboriginal Health Planning Fora (RAHPF) operate in every Western Australian region. The SWAHPF is committed to working on targeted actions in a collaborative and strategic way across health services, planning, policy, and finance to address health priorities in the South West. The aim of the SWAHPF is to ensure Aboriginal people in the South West are able to enjoy the best health outcomes possible.
