You are invited to road test the draft revised WA Advance Health Directive (AHD) template via an online survey.
The draft revised WA AHD template is being developed by the End of Life Care (EOLC) Program within the WA Department of Health. The project is informed by the recommendations from the Ministerial Expert Panel on Advance Health Directives: Final Report (August 2019) and aims to enable more Western Australians to effectively engage in advance care planning.
During the broad consultation phase in August this year, the EOLC Program sought input into the content and design of the revised AHD template from the WA community. The feedback received has been analysed and incorporated and the further revised AHD is now ready to be tested.
Road test the revised WA AHD form online
You can participate in road testing the draft revised AHD template by completing the online survey.
We would like feedback from people who have completed or are considering making an AHD, and health professionals who assist their patients and clients in making an AHD.
The survey will gather your feedback on the usability of the draft AHD template. The information collected will shape further revisions of the form before the AHD template is finalised.

The survey is expected to take about 15 minutes to complete.
